Hylleholt Familiefokus varetager kontaktperson til barnet\/den unge. Der arbejdes ud fra form\u00e5let med Barnets Plan og m\u00e5lene heri samt barnet\/den unges egne \u00f8nsker og forestillinger.<\/p>\n

Kontaktpersonen forholder sig til barnets\/den unges totale situation og g\u00f8r indledningsvist et stort arbejde med at opbygge en tillidsfuld relation. Kontaktpersonen kan v\u00e6re med til at yde st\u00f8tte til barnet eller den unge p\u00e5 det n\u00e6re, personlige plan, kan v\u00e6re en rollemodel for barnet\/den unge lige som kontaktpersonen kan stille krav til eller stoppe den unge, s\u00e5fremt der udvises uacceptabel adf\u00e6rd. Kontaktpersonen kan lige ledes st\u00f8tte barnet i at bevare og styrke gode sociale relationer til familie, venner, etc. Kontaktpersonen kan ogs\u00e5 v\u00e6re med til at opretholde en god struktur p\u00e5 hverdagen.<\/p>\n

Der skrives status hver 3. m\u00e5ned, som udarbejdes af kontaktpersonen og ud fra kommunens Barnets Plan. Dette kan reguleres alt efter, hvad myndighed \u00f8nsker.<\/p>\n
